IRVINE, CA--(Marketwired - Oct 6, 2015) - After more than three years successfully operating its care model in Renton, WA., to serve dual-eligible Medicare/Medicaid complex medical needs patients in King County, ConcertoHealth (formerly Fidelis SeniorCare) today announced it will be opening two additional Concerto Care Centers in Washington and expanding its partnership with Molina Healthcare in the state.
ConcertoHealth has more than a decade of experience caring for Medicare, Medicaid and complex medical needs patients. The company integrates clinical care with social services to ensure patients and their families receive the individualized support they need to improve health outcomes and peace of mind. Concerto delivers high quality primary and preventive care to patients across and between settings -- from the physician's office, to the nursing facility or hospital, to the patient's home.
The new Concerto Care Centers will be located in Lynnwood, WA., to serve Snohomish County, and Lakewood, WA., to serve Pierce County, and will begin seeing members in early 2016.
"Concerto understands the needs of the dual-eligible population and delivers a very personal and differentiated level of care," said Peter Adler, president of Molina Healthcare of Washington. "This personalized approach keeps our members at the center of care at all times. Concerto also provides an array of other attractive benefits, including extended appointments, home visits and non-urgent transportation."
"We value our partnership with Molina Healthcare, and by opening two additional care centers in Washington, we can expand Concerto's healthcare delivery model to serve more of Molina's growing dual-eligible membership," said Alec Cunningham, CEO of ConcertoHealth. "By bringing healthcare to patients where and when they need it, we eliminate many of the barriers to care for the dual-eligible population."
Concerto is a primary care provider delivering the medical, social and logistical support patients need, when and where they need it. Concerto Care Centers are staffed by physicians, nurses, social workers and medical assistants in addition to providing other social and community services, patient outreach and health education.
