IRVINE, CA--(Marketwired - Jul 28, 2016) - ConcertoHealth and Molina Healthcare today announced a partnership to deliver ConcertoHealth's personalized healthcare experience to Molina Healthcare's dual-eligible Medicare/Medicaid members in the Greater Detroit area. This marks an expanded relationship for the companies, as ConcertoHealth already serves Molina's growing dual-eligible membership in the state of Washington.
ConcertoHealth has extensive experience caring for Medicare, Medicaid and complex medical needs patients. The company integrates clinical care with social services to ensure patients receive individualized primary and preventive care, across and between settings -- from the physician's office, to the nursing facility or hospital, to the patient's home.
The partnership will serve members in Molina Healthcare's Dual Special Needs Plan, Medicare-Medicaid Plan, Medicare Advantage, and standalone Medicaid (for recipients aged 50+) plans.
"We are pleased that ConcertoHealth is now a part of our provider network," said Christine Surdock, president of Molina Healthcare of Michigan. "We are delighted to work with ConcertoHealth knowing they are also dedicated to making sure our members benefit from access to quality, coordinated health care."
"We value our partnership with Molina Healthcare and we're excited to expand the relationship to provide improved access to quality healthcare for the dual-eligible population in Greater Detroit," said David Tigue, president, ConcertoHealth of Michigan.
