RITA & ALEX HILLMAN FOUNDATION ANNOUNCES LATEST FUNDING OPPORTUNITY AND RFP FOR THE HILLMAN INNOVATIONS IN CARE PROGRAM

Program seeks grant applications from transformative, nursing-driven models of care serving vulnerable populations


New York, NY, Jan. 22, 2018 (GLOBE NEWSWIRE) -- Continuing its commitment to nursing-led innovation that improves care for vulnerable populations, the Rita & Alex Hillman Foundation today released its latest Request for Proposals (RFP) for the Hillman Innovations in Care Program. Launched in 2014, the Hillman Innovations in Care Program offers two three-year grants of up to $600,000 each. The program seeks applications from nursing-driven, evidence-based, patient- and family-centered initiatives that challenge conventional strategies, improve health outcomes, lower costs, and show potential for national replicability.

The Hillman Innovations in Care Program is one of the Foundation’s defining initiatives and supports programs that improve the health and health care of vulnerable populations, including the economically disadvantaged, racial and ethnic minorities, LGBTQ people, the homeless, rural populations, and others.

RFP Timeline and Details
The deadline for applications is Monday, March 5, 2018. The Foundation will hold an informational webinar on February 13 for those interested in applying. Full details are in the RFP, available here

The Rita & Alex Hillman Foundation specifically seeks proposals that address the health care needs of vulnerable populations in the areas of maternal and child health, care of older adults, and chronic illness management. Priority will be given to proposals that include one or more of the following:

  • Engagement of patients, families, caregivers, and community organizations
  • Inter-professional or multidisciplinary collaboration
  • Institutional and community partnerships
  • Provision of care in non-hospital settings
  • Strong preliminary evidence
  • Measurable goals and outcomes

The Foundation believes that transformative ideas are as likely to come from community health clinics as they are from major academic research centers, and welcomes applications from organizations across the spectrum of care and from practitioners with a range of backgrounds.
