The event brings together health care executives, clinicians and IT professionals to collaborate and exchange ideas for advancing patient engagement, improving clinical outcomes and managing population health across the care continuum.

Approximately 600 members of the IPC community will share the latest patient engagement strategies, learn from hands-on demonstrations and workshops, and connect with industry leaders. The conference will feature more than 70 client-led presentations, community discussions, e-poster presentations, learning labs and more.

National Health Care Leaders and Speakers to Address Patient Engagement

Many of the nation's leading health care organizations -- multi-campus health systems, pediatric institutions, and academic, Military Health and U.S. Department of Veterans Affairs (VA) medical centers -- are scheduled to attend and present at GetConnected 2014.

The conference features case studies, strategic discussions and learning tracks in three distinct areas of focus: Clinical, Strategy and IT.

Clinician attendees will hear proven strategies for advancing the role of nurses as care managers across settings and improving patient outcomes. IT professionals will discuss strategies for interoperability and how true integration of data and workflows enables a more effective patient journey from treatment to wellness. Executive, clinical and IT leaders will examine proven methods for defining and incorporating patient engagement as a core component of an organization's overall strategic plan.

And two world-class hospitals in Chicago will open their doors to GetConnected 2014 attendees and host specially arranged on-site visits.

The University of Chicago Medicine's Center for Care and Discovery will take registered attendees through their 10-story "hospital for the future" and demonstrate how they are leveraging IPC technology to facilitate falls prevention, discharge planning and pain management. Ann & Robert H. Lurie Children's Hospital of Chicago, one of the nation's top 10 children's hospitals, will show registered attendees how IPC has become a central part of the patient and family experience throughout the facility.

Finally, the community's achievements will be recognized during a special awards ceremony.
