PASADENA, Calif., Feb. 11, 2014 (GLOBE NEWSWIRE) -- Kindred Healthcare, the nation's largest provider of post-acute care, is establishing an interoperable IT framework using a single patient identifier assigned by NextGate's Enterprise Master Patient Index (EMPI) to help manage and coordinate care transitions. This is an important part of Kindred's strategic initiative to create integrated-care markets across the country. The enterprise patient identifier supports accurate and seamless data sharing, across the Kindred care continuum, and facilitates connectivity with community Health Information Exchanges and Accountable Care Organizations.
With sites of service across the country that include transitional-care hospitals, skilled-nursing facilities, inpatient rehabilitation facilities and more than 100 home health and hospice locations, Kindred is deploying an internal, enterprise-wide, Health Information Exchange platform to share patient information throughout the patient care cycle.
