IRVINE, CA and DETROIT, MI--(Marketwired - Oct 26, 2015) - ConcertoHealth (formerly Fidelis SeniorCare) today announced it has expanded its footprint in Michigan by opening three additional Care Centers in the greater Detroit Area within 30 days, bringing improved access to healthcare for the complex medical Medicare/Medicaid patient population.
Patients with complex medical needs or dual-eligible status, represent about 20 percent of the U.S. Medicare population and in Michigan total more than 290,000 people.i,ii ConcertoHealth has more than a decade of experience caring for the Medicare, Medicaid, frail and complex medical needs patient populations. It was the success of ConcertoHealth's model that led to the company's selection by the State of Michigan Duals Demonstration Medicare-Medicaid Plan program.
"Our care model delivers individualized clinic services for those who can get to one of our Care Centers, and in-home services for those who can't. We have field-based care teams that follow our members through any other healthcare setting, from hospitals to nursing homes to rehab," said Dave Tigue, President, Concerto of Michigan. "Part of our uniqueness is that by having fully-employed providers that can serve our members in any of those settings, we overcome the inherent risks of transitions between settings and continuity of care."
ConcertoHealth integrates clinical care with social services to ensure patients and their families receive the individualized support they need to improve health outcomes. The company gained a deep understanding of the complexities involved with healthcare delivery and financial risk management of this patient population during its long tenure operating as both a Medicare Advantage health plan (formerly Fidelis SecureCare) and as a care provider through the Fidelis SecureHome medical centers. ConcertoHealth divested its insurance assets earlier this year to focus solely on expanding its medical home and care coordination model.
Concerto's four Detroit Care Centers are located in Detroit (7430 2nd Ave #210, Detroit, MI 48202), Allen Park (15606 Southfield Road, Allen Park, MI 48101), Center Line (25373 Van Dyke Ave., Center Line, MI 48015) and West Detroit (15560 Joy Road, Detroit, MI 48228).
In addition to providing better access to care, ConcertoHealth's new Care Centers have created an infusion of 200 new jobs and infrastructure to the Detroit area.
"We are grateful for the support of Detroit Mayor Mike Duggan's office, together with other local city officials, and we are proud to have created jobs and opportunity for the greater Detroit area," said Tigue.
The ConcertoHealth model includes:
- Care Centers staffed by physicians, nurses, social workers, medical assistants, and pharmacy staff, in addition to social and community services support, patient outreach and health education;
- Care Coordination for each patient available 24-hours, 7-days a week, including in-home care for non-ambulatory patients, bedside patient management in-hospital or post-acute settings, and management of transitions between care settings;
- Chronic Disease Management and comprehensive preventive screenings, access to non-emergent transportation, as well as prescription assistance, the absence of which are key barriers to healthcare services.
"Our mission is to serve a neglected population," said Tigue. "We're an organization that is changing the way healthcare is practiced for complex medical needs patients in Michigan and nationwide."
