The Disease Management performance measure software certification was developed by NCQA in 2012. MedHOK's 360Care® NCQA DM performance measure certified software will help health plans and other entities improve disease management programs by identifying and stratifying eligible populations and constantly monitoring the certified performance measures for each member in the disease management population group. MedHOK was certified for the administrative portion of NCQA's DM performance measures, which are:
- Management of People With Heart Failure
- Management of People With Ischemic Vascular Disease
- Persistence of Beta-Blocker Treatment After a Heart Attack
- Management of People With COPD
- Management of People With Asthma
- Management of People With Diabetes
NCQA became the first DM accreditation organization to utilize performance measures to assess the impact of programs on care for people with asthma, diabetes, chronic obstructive pulmonary disease, heart failure, and ischemic vascular disease.
Achieving three NCQA certifications places MedHOK in an elite group of software companies certified on HEDIS®, P4P and DM. MedHOK's integrated care, quality and compliance platform facilitates improved clinical outcomes and care coordination while enhancing quality measures, compliance, and financial performance. Utilizing NCQA HEDIS® and DM performance measures allows plans holistically to identify disease states, co-morbidities, and care gaps. NCQA P4P helps drive provider focus on quality gaps and improve outcomes.
"NCQA is the gold standard in terms of determining quality in the healthcare arena. These certifications and MedHOK's single platform for care, quality and compliance will help health plans and providers to improve care and quality outcomes," said Anil Kottoor, president and CEO, MedHOK.
