The Wellframe Care Management Platform integrates mobile technologies with artificial intelligence and a supportive service to extend the provision of clinical care from the hospital to the home, empowering patients to better manage their health and helping providers thrive in an evolving payment landscape. The Wellframe Heart Coach seeks to improve post-acute care for patients with coronary heart disease and congestive heart failure, leading causes of mortality and drivers of healthcare costs. In addition to the Wellframe Heart Coach, the company is currently working to expand its clinical program offerings into other clinical verticals such as diabetes and mental illness.
