"Oftentimes, an older adult is seeing four, five, or six physicians," Dave Zientarski, Kendal at Home director of care coordination, says. "It's impossible for [the physicians] to know all the medications you are taking without [the physicians] being physically there when you're taking them." Older adults must be prepared to take an active role in their medication treatment plan to help reduce their risk of medication mismanagement.
The download offers easy tips older adults can follow to get involved in their medication treatment. Also included in the download are:
