Matrix Vice President of Chronic Care Heidi Wold, MSN, ARNP, ANP-BC, will co-present with Vice President and Chief Medical Officer of Government Programs Paul Alexander, MD, MPH, of Horizon Healthcare Services, Inc., New Jersey’s oldest and largest health insurer. The session, Social Determinants in Vulnerable Populations: Impact on Medical Management, will take place on Oct. 21 at 11:30 a.m.
During the session, Ms. Wold and Dr. Alexander will discuss how social determinants affect members’ health and the importance of continuity of care in mitigating these impacts. High-utilizing members disproportionately suffer from behavioral issues, access to care, financial challenges, mental illness and other hurdles.
