With population health management strategy powered by Medecision's Aerial™ platform, Dr. Steinberg will present on the results and evolution of Christiana Care's Bridging the Divides population health initiative, including improved patient outcomes and an annual cost savings of $1.75 million.
Christiana Care's Bridging the Divides program is a CMS-funded platform initiative aimed at improving outcomes and lowering cost in the organization's ischemic heart disease population. Based on lessons learned from the first three years of the program, the health system has expanded the initiative and debuted an advanced technology-enabled care coordination platform within the last year. Dr. Steinberg will discuss how the program, powered by rich patient-level data sets; regressive and predictive analytics; automated care management functions; and risk scoring capabilities from Medecision's Aerial platform has enabled more precise care of the organization's high-risk populations.

Results of the program showed CCHS areas of strengths and weakness in their pilot initiative. These key results now inform a second phase of the organization's population health efforts, including:
- Enhanced patient satisfaction, including 91 percent of patients receiving the right amount of follow-up after discharge and 88 percent were more optimistic about their future;
- A streamlined care management process; and
- The need to expand the program into additional high-risk populations to measure larger numbers of patients.
